Tuesdays with Benefits: Now That Open Enrollment Has Closed

Open Enrollment Has Closed - What's Next?

Remember, if you did not engage in the Open Enrollment process, your 2026 elections will continue into 2027 EXCEPT for the Health Savings Account (HSA) and Flexible Spending Account. Those accounts must be elected every year.

Look Out for These Communications

Employees who made any of the following elections during Open Enrollment (Oct. 12- Oct. 30) will receive an email on Friday, Nov. 6 detailing the next steps for completing and submitting forms online via the DynamicForms process.

Affidavit of Alternative Coverage

If you elected to opt out of both medical and dental coverage for the 2027 benefits plan year, you will receive an email with a link to the Affidavit of Alternative Coverage form. Please complete and submit the form by Friday, Nov. 20, 2026, to receive the opt-out incentive payment. If the form is not received by the deadline, the opt-out incentive will be paid once the form is received. Retroactive payments will not be processed.

Affidavit of Working Spouse/Domestic Partner

If you elected to cover a spouse/domestic partner, a working spouse/domestic partner fee will be applied each pay for non-bargaining and AFSCME employees. The fee can be waived if the spouse/domestic partner meets any of the criteria listed on the form. You will receive an email with a link to the Affidavit of Working Spouse/Domestic Partner form. Please complete and submit the form by Friday, Nov. 20, 2026. If the form is not received by the deadline, the surcharge will be applied beginning January 2027 and until the form is received. Retroactive refunds will not be processed.

Evidence of Insurability (EOI)

If you elected to add or increase supplemental life insurance for yourself or your spouse, you may need to complete the Evidence of Insurability (EOI) documentation. If required, you will receive an email with a link to the form. Please complete and submit the form by Friday, Nov. 20, 2026. NOTE: You will not need to complete the EOI form if you have been pre-approved for any amounts over the guaranteed issue –$200,000 for supplemental life for yourself and $20,000 for supplemental life coverage for your spouse/domestic partner – or you requested additional supplemental life insurance coverage during Open Enrollment that was not over the guaranteed issue amounts.

Health Savings Account Enrollment

If you are enrolled in the High Deductible Health Plan for 2026 and will continue that plan into 2027, but did not engage in the Open Enrollment process, you will receive an email with a link to the Health Savings Account Enrollment form. Please complete and submit the form by Friday, Nov. 20, 2026.

Managing Diabetes? Don't Miss Out on This Program

Medical Mutual offers a Chronic Condition Management Program designed to help members manage diabetes and stay on track with their health goals. One of the valuable features of this program is access to eligible diabetes supplies with a $0 copay. To maintain eligibility for the $0 copay, enrolled members must complete a brief quarterly wellness check-in with Stephanie, Medical Mutual's AI Care Navigator.

Why am I receiving phone calls?

If you are enrolled in the program, you will receive a phone call from Stephanie once every three months. These calls are part of the program's wellness check-in process and are required to maintain your eligibility for the $0 copay on eligible diabetes supplies.

What if I don't recognize the number or the call is marked as spam?

Medical Mutual has worked to help ensure these calls display as Medical Mutual on your caller ID. However, some phone carriers may still identify them as potential spam. Here are a few helpful tips to make sure you don't miss your call:

  • Sign up for text notifications: Log in to your Medical Mutual My Health Plan portal and opt in to receive text messages. You may receive a text letting you know Stephanie will be calling soon.
  • Check your voicemail: If you miss the call, Stephanie will leave a voicemail. Make sure your voicemail box isn't full so you can receive the message.
  • Return the call: You can call Stephanie back at the number provided in the voicemail. You have two weeks from the initial call to complete your check-in. Calls can be returned Monday–Friday, between 8:30 a.m. and 8 p.m.

Not enrolled or have questions about getting started?

Medical Mutual can help you determine which Chronic Condition (Total Health) Management Program may be right for you based on your health condition and personal goals.

Call the Medical Mutual Population Health team at 1-800-590-2583 to learn more about the program and enrollment.

Take advantage of this valuable benefit! Staying connected with the program helps you access support for managing your diabetes while maintaining eligibility for $0 copays on eligible diabetes supplies.
